Learning to Live Well with a Chronic Condition
American Sign Language Interpreters will be providing the translation of spoken language into sign language during the six- week program.
Tuesdays, April 2, 9, 16, 23, 30; May 7, 2024

The Chronic Disease Self-Management Program (CDSMP) is peer-led health program. It is for adults 18+ with any ongoing health issue. This can be asthma, COPD, arthritis, etc.
CDSMP helps each person manage their health. Classes are once a week for six weeks.
Each person will receive a book and resources.

Participants must be 18 years of age and live in Onondaga County.
Voluntary contributions to the Office for Aging are accepted and used to expand services.
No one will be denied services if unable or unwilling to contribute. A contribution in any amount is welcomed. Please make checks payable to Upstate OASIS. Thank you.
